Consult your Collective Bargaining Agreement for S&A benefit parameters, then contact your Human Resource Department for the appropriate paperwork if you are going to be absent from work for a medical reason. Complete the appropriate sections and be sure to have your physician complete the applicable sections. Return the paperwork to HR ASAP to avoid any delay in your benefits.

Sickness & Accident benefits begin on day one for an injury; however, there is a seven day waiting period for an illness.
